Job Description:

LPN’s provide consistent high-quality care in a Health Care Setting, by identifying resident needs and progress, medical assessments and promotion of team approach to resident care. Provide support to our CNAs with resident care and charting.

Duties Include:

• Follow recommended safety procedures and practices, and proper nursing procedures.
• Complete documenting, receiving and giving reports including 24 hour assessments & charting.
• Manage medication and provides treatment for residents.
• Notify family of change in condition & new orders
• Communicate & follow up with therapy recommendations.
• Daily assess & manage resident care/needs.
• Supervise CNA’s assigned to your shift.
• Serve/participate on clinical care committees as assigned.
• Communicate with resident’s physician regarding resident’s (and/or change of) health care needs.

Job Requirements/Qualifications:

• Licensed as a Licensed Practical Nurse by the State of Indiana. Must maintain license as required by the facility or the state.
• Three years of experience working LTC preferred

Schedule:

• Part time and PRN available
• Hourly rate based on experience
• $1.50 additional per hour for second or third shift.
• Weekend shift pay: $1.50 additional per hour
